The Denver Health Medicare Choice (HMO D-SNP) plan is for you if you have Medicare Parts A & B and full Medicaid benefits. In addition to original Medicare benefits, coverage includes:

  • 24/7 NurseLine to answer your questions and call in certain prescriptions

2020 Denver Health Medicare Choice (HMO D-SNP) Pharmacy

  • Part D prescription drug premium: $31.30, which may be paid by “Extra Help", also known as Low Income Subsidy (LIS).
  • Annual pharmacy deductible: $435 or less, depending on LIS.
  • Premiums, copays, coinsurance and deductibles may vary based on the level of “Extra Help” the beneficiary receives. Please contact a sales agent for more information.
