Medicaid and CHP+ Members

Take a moment today to make sure that Health First Colorado (Colorado’s Medicaid Program) and Child Health Plan Plus (CHP+) have your most recent phone number, email and mailing address. It is vital that Health First Colorado or CHP+ can reach you. There may be steps you need to take to renew your coverage. Members will be sent a renewal packet to sign and return. This can be done by mail, on a computer or over the phone.

You can change your Medicaid or CHP+ information one of these ways:

  1. Visit Colorado.gov/PEAK
  2. Use the Health First Colorado app on your cell phone. This free app is for Health First Colorado
  3. Medicaid and CHP+ members
  4. Medicaid and CHP+ members call 800-359-1991
  5. Reach out to your county Department of Human Services
  6. Reach out to your local Medical Assistance Site (Medicaid and CHP+)

Welcome to Elevate Child Health Plan Plus

Elevate Child Health Plan Plus (CHP+) is a low cost health plan for children from birth through age 18. Children who are approved for CHP+ must enroll in a health plan and must not have any other insurance. Children who live in Denver, Jefferson, Arapahoe, or Adams counties can enroll in Denver Health Medical Plan, Inc. (DHMP) as their health plan.

As a DHMP member, you can get care at the Denver Health main campus and any of Denver Health’s 10 Family Health Centers throughout metro Denver, as well as various Denver Health School-Based Health Centers. In addition, CHP+ members enrolled with DHMP pay no copays for covered visits and medicines and have expanded benefits including eyewear and hearing aids.
