Welcome to Elevate Child Health Plan Plus
Elevate Child Health Plan Plus (CHP+) is a low cost health plan for children (through age 18) and who live in Denver, Jefferson, Arapahoe or Adams counties. Children who are approved for CHP+ must enroll in a health plan and must not have any other insurance.
Members get care at the Denver Health main campus and any of Denver Health’s Family Health Centers in metro Denver, as well as various Denver Health School-Based Health Centers. Elevate CHP+ members pay no copays for covered visits and medicines and have expanded benefits, like eyewear and hearing aids.

MyDHMP is your go-to resource for managing your health insurance plan anytime, anyplace.
Access benefit information, download member materials (including ID cards), communicate with health plan staff, check claim status and more — all right from your desktop, tablet or smartphone.

| Non-Urgent, Non-Symptomatic Care Visit | Within 30 days |
| Non-Urgent, Symptomatic Care Visit | Within 7 days |
| Well Care Visit | Within 1 month* |
*Unless an appointment is required sooner in accordance with the department's accepted Bright Futures Schedule.
Taking Care of Your Child's Health
It's important for you child to get a yearly well-child exam to stay healthy and informed. Talk to your health care provider about your child's physical and behavioral health needs.
