DHMP reserves the right, in its sole discretion, to terminate your access to the portal at any time without notice for any reason whatsoever.

Security and Confidentiality

DHMP is committed to protecting the confidentiality of your medical information. DHMP affords the same degree of confidentiality to medical information accessed via this Website as is given to medical information controlled by DHMP and accessed in any other medium. Firewalls, passwords, encryption, and audit trails are all used to safeguard your information.

If you are an authorized user of the member/provider portal, your username and password (your "Login Information") will allow you to access certain information about you and your plan. You are solely responsible for controlling your Login Information and for authorizing, monitoring and controlling access to your account. You agree to notify DHMP immediately of any unauthorized use of your Login Information or of any need to deactivate the Login Information associated with your account.
