Qualify to receive a $25 reward following completion of your Health Risk Assessment.
- We will call you to complete the Health Risk Assessment over the phone or we may mail it to you to fill out. You can also call us to complete the HRA at 1-833-292-4893. TTY users should call 711.
- You may use this reward to purchase healthy food items. Members qualify after completing the HRA. Funds expire March 31 of the year after the reward was earned.
- Your reward will be loaded onto your FlexCard within within one month of completing the HRA. See FlexCard frequently asked questions (FAQs) for more information.

About the Health Risk Assessment
Each year, we will ask you to complete an HRA, so we know if there were any changes in your health. By answering the questions on the HRA, we can identify your health risks and help you take action to reduce them.
Your answers to the HRA questions do not affect your insurance coverage and may be shared with your doctor. We can work with your doctor and health care team to help you improve your health.
