For On-Exchange Members (ACA Marketplace)

Plans purchased through Connect for Health Colorado

Members who purchased their plan through Connect for Health Colorado (on-exchange plans only).

Form 1095-A:

  • Shows the months you (and other members of your household) were enrolled in a health insurance plan last year
  • Shows how much you paid in monthly premiums and how much tax credit (if any) you received last year to pay for your health insurance
  • Helps you fill out Form 8962. Form 8962 determines how much tax credit you qualified for based on your actual income and household size as shown on your federal income tax return. 

Connect for Health Colorado mails a copy to the primary tax filer at the end of January. You can also download an electronic copy from the "My Documents" section of your Connect for Health Colorado online account.

For Off-Exchange Members (DHMP/CO Connect)

Plans purchased directly through DHMP or CO Connect

This form reports that you and your dependents had qualifying health coverage (Minimum Essential Coverage). Note: This form is not required for you to file your taxes.

Members who purchased their plan through DHMP or CO Connect. (Off-Exchange Plans Only).

Forms are generally sent by mail by the IRS deadline of January 31.

Please contact Health Plan Services to:

  • Let us know your preferred email address so we can securely send you an electronic copy.
  • Verify your current mailing address if you prefer a hard copy.

DHHA Employees

Employer-Sponsored Coverage

This form confirms to the IRS that your employer (DHHA) offered you affordable health insurance coverage. Note: This form is not required for you to file your taxes.

DHHA employees receive their copy from DHHA HR before the April 15th tax deadline.

Additional Plan Notices

Medicare Members 

Denver Health Medical Plan does not send 1095 tax forms for Medicare. To get a copy of your IRS Form 1095-B, please call Medicare at 1-800-633-4227, TTY users call 1-877-486-2048.

Medicaid/CHP+ Members

You should receive this form directly from the State. If you need another copy, please contact your local Health and Human Services or CMAP at 1-800-359-1991.
