Individual and Family Plans
These plans include Exchange and Colorado Option offerings for individuals and families in Colorado.

Medicare Plans
These plans are for individuals who live in Denver, Adams, Arapahoe or Jefferson County and hae Medicare Parts A and B.
Medicaid Plan
Members who qualify for Medicaid coverage are assigned to Elevate Medicaid Choice by Health First Colorado
CHP+ Plan
This plan is for children from birth through age 10 and pregnant people who are approved for CHP+. Enrollment is through Health First Colorado.
DHHA Employer Plans
These plans are available to DHHA employees and their qualified dependents.

Base Plan
These are the base plans for Exchange Bronze HDHP, CO Option Bronze, CO Option Silver and CO Option Gold.
NA
This plan is for Native Americans.
NAZERO
This plan is for Native Americans with zero-dollar cost sharing.
Off Exchange
This plan is for members who enrolled directly through DHMP or through Colorado Connect.
Off Exchange Enhanced (Colorado Option Silver Only)
This plan is for members who applied through Colorado Connect (OmniSalud program) and selected the ‘Silver Enhanced’ plan option.
73% / 87% / 94% (Colorado Option Silver Only)
These plans include Cost Sharing Reductions (CSRs).
