Denver Health Medical Plan, Inc. is utilizing the same list of Hierarchical Condition Category (HCC) codes recognized by the Centers for Medicare and Medicaid Services (CMS). Below is a full list of qualifying HCCs by condition.

Members who believe they qualify but there is not an eligible HCC on file within the last 48 months, can self-attest. Those members have 12 months to visit their provider and get diagnosed for the codition to continue receiving the benefit.
