Your Vision Benefits Include:
As an Elevate Medicare Advantage member, you have vision benefits through any in-network provider at no extra cost.
- One routine eye exam every year
- Allowance for contact lenses and/or eyeglasses (lenses + frames) every year. Check the Summary of Benefits or Evidence of Coverage for the annual benefit allowance for vision hardware.

Pro Tip: To maximize your benefit, you can get frames or lenses at any provider (such as Costco, LensCrafters, Warby Parker, Zenni, etc.) usually for a lower cost. You can do this and submit to DHMP for reimbursement up to the benefit allowance. See the FAQ below for details.
Cataract Surgery and Eyeglasses
You’re covered for medically necessary cataract surgery that includes a standard intraocular lens and one pair of basic eyeglasses or contact lenses after each surgery.
- Upgraded frames or lens options are not covered by DHMP and will be your responsibility.
- If you have cataract surgery on both eyes at different times, you’ll receive one pair of eyeglasses or contacts after each surgery. You can’t save the first benefit to use later.
- Your post-cataract eyewear benefit is separate and does not reduce your regular vision allowance for prescription glasses or contacts.
- You can get your post-surgery eyeglasses or contacts from an in-network vision provider or request reimbursement from DHMP. However, reimbursement requests must include the correct diagnosis codes related to cataract surgery, so you will need to provide the appropriate medical records. See your current vision allowance based on your plan by reviewing your Summary of Benefits or Evidence of Coverage.
DHMP will reimburse Elevate Medicare Advantage members up to the annual maximum vision allowance for prescription lenses and contacts if purchased at another provider.
DHMP will not reimburse for other vision care (exams, non-prescription eyewear, etc). The claim must be submitted with 12 months of the date of service. Please see the member reimbursement form.
Diabetes may cause an eye condition known as diabetic retinopathy. High blood sugar levels can cause damage to blood vessels in the body, and specifically to the part of the eye known as the retina. This damage can lead to blurry or distorted vision, and in worst-case scenarios, it can lead to complete loss of vision.
The good news is that it may be preventable. With proper blood sugar control and regular retinal exams with an eye care provider, you can stay on top of it. Please make a point to see your eye care provider at least annually, and make sure they know about your diabetes diagnosis.
