You may qualify for the Healthy Food Allowance
- Elevate Medicare Choice (HMO D-SNP)
Up to $310 every quarter (all members qualify) - Elevate Medicare Select (HMO)
Up to $75 every quarter (members with certain chronic illnesses may qualify for the Healthy Food Allowance)

Qualifying Rules for the Healthy Food Allowance (for Elevate Select members only)*
If you have one or more of the following chronic illnesses and you have a paid claim from Denver Health Medical Plan (DHMP) with an eligible diagnosis code that is related to one of the below conditions, you are eligible for this benefit.
Let us know if you qualify by completing the self-attestation form. Your attestation is valid for up to 12-months from this submission. During this time, it is important to see your doctor so they can diagnosis your condition(s).
*Select Members: This benefit is part of special supplemental program for the chronically ill. Not all members qualify. Other eligibility and coverage criteria apply. Eligible conditions include: chronic alcohol and other drug dependence, autoimmune disorders, cancer, chronic heart failure, dementia, diabetes, end-stage liver disease, end-stage renal disease (ESRD), severe hematologic disorders, HIV/AIDS , chronic lung disorders, chronic and disabling mental health conditions, neurologic disorders, stroke.
