General Details

Qualifying allowances are loaded onto a reusable card.

  • Quarterly balance reloads on January 1, April 1, July 1, October 1.
  • Monthly balance reloads on the 1st of the month.
  • Balance does not carry over.

No. If you re-enroll, a new card is not automatically reissued, unless you left the plan for greater than 90 days. If you left for less than 90 days and need a new card, call Health Plan Services at 303-602-2111, toll-free 1-877-956-2111 or TTY 711, 8 a.m. - 8 p.m., seven days a week.

A PIN is not required, but it can secure your card in case it is lost/stolen, so no one can utilize your eligible funds. To set up a PIN call 303-602-2111 and select option 2. Have your card ready to provide when prompted and utilize the automated prompts to set up a PIN.

If the information requested by the IVR to set up your PIN is entered incorrectly, the PIN setup will be locked after three unsuccessful attempts. The lockout will remain in place for 24 hours and will automatically unlock once the 24-hour period has passed.

If you would like to verify whether your PIN setup is locked, please contact Health Plan Services at 303-602-2111. Please allow 24 hours before attempting to set up your PIN again.

You don't need to request a new FlexCard. Approximately 30 days before your current card expires, a replacement card will automatically be mailed to you. Your current card will continue to work until its expiration date. Once you receive your new card, begin using it for eligible purchases and expenses.

If you think you may have misplaced your FlexCard and need time to look for it, call Health Plan Services to temporarily suspend your card. Suspending your card helps protect your benefits from unauthorized use. If you find your card, the suspension can be removed, and you can continue using the same card.

If your FlexCard is lost or stolen, call Health Plan Services to report it. A lost or stolen card cannot be reactivated. A new FlexCard with a new card number will automatically be mailed to you.

Replacement cards typically arrive within 7–10 days. If your current benefit allowance is expiring soon, please keep this timeframe in mind.

While waiting for your replacement card, you may still be able to use your allowance through Medline, where a physical FlexCard is not required.

OTC/food catalog

Yes, the FlexCard can be used out of state; however, the approved store/vendor list still applies. The stores below are approved, but availability may vary depending on the state where you will be using your card.

  • Safeway
  • King Soopers
  • Costco
  • Walmart
  • CVS
  • Target
  • Walgreens
  • Dollar General

You can also use the merchant locator on the app to find approved stores near you. In addition, if you provide Health Plan Services with the ZIP code for the area where you will be located, they can send you a list of approved stores nearby.

The Shopping Process

If you try to purchase an item/expense with your FlexCard and it declines, you may submit it for review. Simply fill out the FlexCard Declined Submission form. Failure to provide all requested information will result in no review of the declined transaction(s), due to not enough information to troubleshoot.

Sometimes, a transaction may be declined because an otherwise eligible item or expense is not included on the Approved Product List (APL) for Healthy Food and OTC purchases or does not fall under an approved Merchant Category Code (MCC) for expenses such as utilities, transportation, or gym memberships.

If you believe an item or expense is eligible and would like Elevate Medicare Advantage to consider adding it to the approved list, you may submit a FlexCard Declined Submission Form with information about the item or expense.

Please allow up to 45 days for the item or expense to be reviewed. Submitting a request does not guarantee that the item or expense will be approved or added to the list.

The FlexCard mobile app will allow you to scan an item to confirm if it's eligible. Download the Elevate Medicare FlexCard mobile app with your Android or iPhone.

If you don't want to use the app, you can:

  • Purchase items and qualifying items will automatically be covered with eligible FlexCard funds when you check out
  • Place your order with Medline by phone or by mail using the OTC/food catalog.
  • If you want a small list of approved SKUs for a specific item to look for in store, you can call Health Plan Services to email you or mail you a list.

Yes, and you can check out all your items out at once. Once it is time to pay, you will swipe your Elevate Medicare FlexCard and the card will automatically pay for any eligible items up to your eligible allowance. The system will then ask you for another form of payment for any non-eligible items or items above the balance on your Elevate Medicare FlexCard.

No, you should swipe it like a regular MasterCard and it will pay for eligible items up to the balance on your card. After it has finished applying the funds, it will ask you for another form of payment for the remaining balance.

The FlexCard should be used first because the EBT (SNAP) card may cover additional items. Or you can pull out items you know are covered by the FlexCard and purchase those, then do a separate transaction using your EBT (SNAP) card.

Transactions may take up to 7 business days to settle. If the store does not settle or reverse the transaction by the end of the 7th business day, the funds will be returned to your account on the 8th business day.

Note: If there are 7 or fewer days remaining in the month when the funds are due to be reversed, the monthly allowance will be forfeited. Please keep in mind that funds expire at the end of the month/quarter. We encourage you to plan accordingly.

In some cases, the funds may be returned to your FlexCard account while the transaction continues to show as pending. This can happen when the store does not complete or settle the transaction. If the funds have been returned to your account, no action is needed.

Fraudulent Activity and Disputes

If you notice an unauthorized transaction, you will need to file a dispute using the Dispute Form located in your FlexCard member portal under Tools & Support. Have the date and amount of the transaction ready before completing the form. Once finished, send the form to the address provided on the document. If your dispute is approved, the FlexCard vendor will credit your account for the approved amount.

*Note: The dispute must be sent within 110 days of the transaction date and it must be a settled/completed transaction.

Call HPS to report your current card as lost/stolen as soon as possible to avoid additional fraudulent activity.
WEX will also set a fraud flag once the dispute is received. And the card won’t be able to be used.
Failure to report the card as lost/stolen can also delay the dispute process

It depends on whether the refund is from the same plan year.

  • Refunds and credits from disputes carry forward to future benefit periods within the plan year.
  • Refunds and credits from disputes will continue to be available through the end of the plan year.
  • Refunds and credits from disputes will not automatically roll over into the next plan year.

Portal/App

For additional security measures, the app or web portal may request multi-factor authentication. The system will send an additional password to your mobile phone or email on file in some scenarios like the first time you register for the portal or on a new device, if you request a new card, or if you change your password. If you need to update your phone number or email, please contact our Health Plan Services at 303-602-2111 (TTY 711) and talk to a representative.

*The Healthy Food and Utilities Allowances offered with the Elevate Medicare Choice (HMO D-SNP) plan are part of special supplemental program for the chronically ill. Not all members qualify. Other eligibility and coverage criteria apply. Eligible conditions include cancer, chronic heart failure, diabetes, chronic lung disorders, and chronic and disabling mental health conditions. Other conditions also apply that are not listed.
