The Importance of Getting Dental Care
Your oral health is connected to your overall health. Your dentist can spot early signs of heart disease, diabetes and other diseases. Make preventive dental care a priority by visiting your dentist twice per year.
As an Elevate Medicare Advantage member, you have dental benefits through Delta Dental of Colorado. Your dental benefit allows you to see any Delta Dental Provider in the PPO Medicare Advantage Network. Delta Dental has many dentists in your area.
Your Dental Benefits
Members receive the following services at no cost through an in-network dental provider. Benefits restart January 1st for each calendar year.
- Two oral exams and cleanings
- One fluoride treatment
- Bitewing X-rays

Getting Care
Members have access to any dental provider participating in the Delta Dental Medicare Advantage PPO Only Network.
Important Information for Health First Colorado/Elevate Medicaid Members:
Your provider must also accept DentaQuest through Medicaid. Ask your provider if they are in network with Medicaid and the Delta Dental PPO Medicare Advantage Network.
When looking for a dental provider, make sure they meet your needs for things like location, hours and accepting your insurance. Ask for recommendations from friends, family and neighbors to help find a provider that is the right fit for you.
For in-network dental providers:
- Call the Denver Health Dental Clinic at 303-436-4949.
- Find a Delta Dental Medicare Advantage PPO Provider by calling 1-800-610-0201 or visiting the Delta Dental provider directory online.
- Download the Delta Dental mobile app on your Android or iPhone. The app makes it easy to search for a dentist near you, view ID cards and more!
The dental office will need your Denver Health Medical Plan, Inc. health plan ID. This usually starts with an MA or a 7000. If your ID starts with 7000, the dentist needs to add a leading 0 to the ID so they can find you. If they cannot find your account, they can contact Delta Dental of Colorado at 1-800-610-0201.

Help With Costs
Dental care can be expensive, but there may be resources available to you when the cost is more expensive than the plan benefits or if a service isn’t covered by your plan. Seniors with low income may qualify for the Colorado Dental Health Care Program. To qualify, you must be:
- Age 60 and older
- Income at or below 250% of the current federal poverty level for household size
- Cannot be eligible for Medicaid, Old Age Pension Medical, or have private dental insurance (but can be on Medicare)
- Colorado resident
You may be asked to provide certain documentation to confirm your eligibility, so remember to call ahead and gather the necessary documents. Some services that may be covered include:
- Oral examinations
- Diagnosis
- Treatment planning
- Emergency treatment
- Prophylaxis
- X-Rays
- Partial and full dentures
- Removal of permanent teeth
- Fillings
- Periodontal treatments
- Soft tissue treatments
- Sedation
