CONTRACT CHANGE REQUEST FORM

Use this form for existing contract addendums, including location changes, NPI or TIN updates and requests for contract rate reviews. Business acquisitions are subject to new contract terms and conditions.

FACILITY CREDENTIALING FORM

You must have a completed contract agreement with DHMP in order to credential facilities. New locations should be added by addendum to your contract.

DHMP PROVIDER NETWORK

The Denver Health Medical Plan provider network is currently closed to new contract agreements. 

We routinely review and evaluate network needs.
