Here's How Check-Ins Work

You'll receive a notification once a week that takes 3 minutes or less to complete. You have the option to snooze and complete it later, if needed. After answering a few questions, you can explore helpful tips, videos, or links. Topics include:

  • Chronic condition support (like diabetes or high blood pressure)
  • Medication reminders
  • Mental and emotional health
  • Preventive care
  • Nutrition, exercise, and healthy habits
  • Recovery after a visit or hospital stay
  • Health plan benefits and how to use them
  • Resourceful links to help optimize your health journey
