Your Elevate Medicare Advantage Plan by Denver Health Medical Plan, Inc. (DHMP) is offering qualified members an annual checkup or routine care, at no cost, through Focus Care. Focus Care is a national medical group that provides primary care to you in your home.
This benefit is available to both Elevate Health Plan (Exchange/CO Option) members and Elevate Medicare Advantage members (Medicare Advantage members may qualify for a $30 Annual Wellness Visit Reward toward the purchase of healthy food after completion of an In-Home Assessment (IHA) checkup.

The Advantages of an In-Home Assessment
With an In-Home Assessment, you can get care in the comfort and safety of your own home. Focus Care will schedule a nurse practitioner to discuss your health, review your medications, close care gaps and address any concerns. You can even schedule these visits on nights and weekends. We want to make sure you get the care you need.
Frequently Asked Questions
An In-Home Assessment provides one-on-one time with a licensed nurse practitioner. The nurse practitioner will perform an exam, review health history, and answer any health-related questions.
If you are selected to participate in this program, you will be contacted by a representative from Focus Care.
If you are a member of Elevate Medicare Advantage, we will be contacting you at the beginning of the year, and we will continue to conduct outreach.
You will be contacted at the phone number and address listed on your account to schedule a visit. You will receive a letter in the mail with instructions and a phone call. Calls occur Monday through Friday between the hours of 8 a.m. to 5 p.m. Mountain Time.
The In-Home Assessment allows you to speak with a nurse practitioner and ask health-related questions in the privacy of your home. The In-Home Assessment does not affect benefits and there is no extra cost.
- A quiet place with few distractions
- Loose-fitting, comfortable clothes to make the physical exam easier
- All of your medications, to show the nurse practitioner.
This visit can be in person or it can be a virtual health visit in the comfort of your home, at a time that is convenient for you. There is no cost for the In-Home Assessment.
If you prefer not to have the nurse practitioner come into your home, you can schedule an appointment to complete your checkup via phone and video. To complete a virtual In-Home Assessment:
- You will need a smart phone, tablet or computer with a speaker, microphone, camera and internet access.
If you use an Apple smart phone, tablet or computer, we recommend using the Safari internet browser.
For all other smart phones, tablets and computers, we recommend using the Chrome internet browser. - A few minutes before your scheduled visit, your nurse practitioner will call you and email or text you a link to join the telehealth visit.
- Join with your phone, tablet or computer and speak with the nurse practitioner as though you were in an exam room at your doctor’s office.
- The nurse practitioner will ask you questions about your health and medical history.
- You will also be asked to move the camera around so the nurse practitioner can examine your body, including your eyes, tongue and chest.
