1. Sign up for your MyDHMP portal account. MyDHMP is your go-to resource for managing your health insurance plan from your desktop, tablet or smartphone. Access benefit information, download member materials (including ID cards), communicate with health plan representatives, check claim status and more — all right from your desktop, tablet or smartphone. You can access MyDHMP on your computer or tablet or download the MyDHMP Portal App for your smartphone.

2. Explore your network providers by following the Find a Provider link at the top our website or in your MyDHMP portal account. Schedule an appointment to establish care if you are new to the plan. If you’re already a member, be sure to schedule your annual preventive care visit with your Primary Care Provider (PCP). 

3. Set up your medications. As a member, you have multiple ways to fill your medications. Refer to the Pharmacy page for your plan for details.

Yes. You can call call Health Plan Services. The Health Plan Services telephone number can also be found on your member ID card. It can help to write the number down or save it to your phone so you have it handy if you lose your card.

Covered services are listed in your Member Handbook (also called an Evidence of Coverage/EOC).

  • You can access your Member Handbook/EOC in the MyDHMP portal, under 'Benefit Documents'.
  • You can also find your Member Handbook/EOC on this website. Look for 'Members' at the top of the page, select your plan and find Member Handbook or EOC on your plan page.

Please refer to your Member Handbook/EOC for the cost of services and provider visits.

  • Prescriptions can be filled at any National Network Pharmacy, such as King Soopers, Walgreens, Safeway and most large retail pharmacy chains.
  • Prescriptions can also be filled at any Denver Health Pharmacy (as long as they are written by a Denver Health provider). Denver Health Pharmacies will give you the lowest prescription costs. 
  • Use our online tool to search drug prices by visiting your plan’s Pharmacy page. The tool will give you drug prices as Denver Health as well as at national network pharmacies. 
  • For a list of all in-network pharmacies, refer to the Pharmacy Directory found on your plan pharmacy page.

Prescriptions typically fall into different “tiers” or cost levels. You can find more information about cost of drugs in your Member Handbook, and specific drugs and their tiers are listed in your plan’s drug formulary. Also found on your plan’s pharmacy page. All of this information can be found in your member portal account or on our website. 

We are required to send you some information by mail, but you can opt-in to certain communications and receive them by email.

You can pay your premium by enrolling in monthly autopay or by making a one-time payment. Look for the Pay My Premium button at the top of our website to pay your premium and learn about your premium payment options.

 

Elevate Medicare Advantage is a Medicare-approved HMO plan. Enrollment in Elevate Medicare Advantage depends on contract renewal. The Plan also has a written agreement with the Colorado Medicaid Program to coordinate your Medicaid benefits.
