What is Health Coaching? What can Health Coaching do for me?
Health Coaching is a NO-COST program to help you take a more active role in your health care! Health coaches will help boost your motivation by encouraging and supporting you in making lifestyle changes to improve your health!
Health Coaching offers self-management support, motivation, encouragement and compassion to you as you address chronic health care needs or make health behavior changes. The services offered through the program are voluntary. To qualify for this program you must be a DHMP Medicare, Elevate or Employer Group (City of Denver/DERP, Denver Police or DHHA) member.
Health Coaches work with you on a variety of health conditions or wellness goals including:
- Congestive Heart Failure
- Healthy Eating
- Medication Adherence
- Pain Management
- Physical Activity
- Smoking Cessation
- Stress Management
- Weight Management
Health Coaches are skilled at helping guide people into action. They will ask questions to get a better understanding of you as a person and the barriers that might be keeping you from achieving your health goals. They help empower you to become a better self-manager of your health.
