Diabetes Support includes:
- Group Education Classes - You benefit from receiving information on a variety of topics geared toward helping you better self-manage your condition while also receiving support from others with whom you can relate.
- 1:1 Telephonic Health Coaching - Health coaching helps you take a more active role in your health care. Health coaches help boost your motivation by encouraging and supporting you in making lifestyle changes to improve your health.
Depression and Anxiety Support includes these self-management support services:
- 1:1 Telephonic Counseling in the Telephonic Counseling for Depression and Anxiety Program - In this program, you will work with a health coach over the phone to learn ways that you can improve your mood. You will be able to focus on how to worry less, have more positive thoughts, do things you enjoy more often, have better relationships and so much more!
- myStrength Online Self-Help Program - You are asked to fill out a wellness profile through a secure online program where your information is kept private. You can access eLearning tools, read articles, watch videos or simply be inspired by daily quotes and pictures.
These self-management support programs are intended to help you better understand your condition, update you on new information, and provide you with assistance from our staff of health coaches and behavioral health clinicians. The program is designed to reinforce your treatment plan.
Eligible patients do not have to enroll in STRONG body STRONG mind programs; you are automatically enrolled when we identify you as having any of these conditions. If you are a DHMP member and would like to participate in the program, please let us know by calling 303-602-2136.
Participation in the programs is voluntary. If at any time you wish to stop participating in the program, you need only to call 303-602-2136.
