Delta Dental

Elevate Exchange is the only exchange health insurance product in the Denver area that offers dental coverage for adults age 19 and above. In partnership with Delta Dental of Colorado, the plan’s covered services include:

• Oral exams and cleanings: twice per calendar year (for individuals with prior definitive periodontal treatment or certain medical conditions, two additional cleanings per calendar year)
• Bitewing X-rays: once per calendar year
• Full mouth X-rays: once per calendar year (unless documentation of special need is provided)

Note: All services must be performed by a Delta Dental PPO provider.
