How to Request a Hard Copy of Provider or Pharmacy Directory

If you need help finding a network provider and/or pharmacy, please call Health Plan Services at 303-602-2111 or toll-free at 1-877-956-2111. TTY users call 711. You may also visit our provider directory and pharmacy directory online.
