Denver Health Medical Plan offers STRONG body STRONG mind Diabetes, Depression and Anxiety disease management programs to support your patients' wellness.
Diabetes Support includes:
- Group Education Classes - Your patients benefit from receiving information on a variety of topics geared toward helping them better self-manage their condition while also receiving support from others with whom they can relate.
- 1:1 Telephonic Health Coaching - Health Coaching helps your patients take a more active role in their health care. Health coaches help boost motivation by encouraging and supporting your patients in making lifestyle changes to improve their health.
Depression and Anxiety Support includes these self-management support services:
- 1:1 Telephonic Counseling in the Telephonic Counseling for Depression and Anxiety Program - In this program, your patients will work with a Health Coach over the phone to learn ways that they can improve their mood. They will be able to focus on how to worry less, have more positive thoughts, do things they enjoy more often, have better relationships and so much more!
- myStrength Online Self-Help Program - Your patients are asked to fill out a wellness profile through a secure online program where their information is kept private. They can access eLearning tools, read articles, watch videos, or simply be inspired by daily quotes and pictures.
These self-management support programs are intended to help your patients better understand their condition, update them on new information, and provide them with assistance from our staff of health coaches and behavioral health clinicians. The program is designed to reinforce your treatment plan.
Eligible patients do not have to enroll in STRONG body STRONG mind programs; they are automatically enrolled when we identify them as having any of these conditions. If they are a DHMP member and would like to participate in the program, please let us know by calling (303) 602-2136.
Participation in the programs is voluntary. If at any time you wish to stop participating in the program, they need only to call (303) 602-2136.
