Child Health Plan Plus (CHP+) is a low-cost health plan for children from birth through age 18. Children who are approved for CHP+ must enroll in a health plan. Children who live in Denver, Jefferson, Arapahoe, or Adams counties can enroll in Denver Health Medical Plan, Inc. (DHMP) as their health plan. As a DHMP member, your child can get care at any of Denver Health's nine family health centers throughout metro Denver as well as 15 Denver Health school-based health centers. In addition, CHP+ members enrolled with DHMP pay no copays for covered visits and medicines and have expanded benefits including eyewear and hearing aids.

How can I sign my kids up for CHP+?
First, make an appointment with an Enrollment representative by calling 303-602-2300. You can download the application and bring it with you to the appointment, or the enrollment representative can fill it out when you meet. At the top of the right hand column is a list of the items you will need for this appointment. When you leave this enrollment appointment you will know what program your children are qualified to join.
